PCPI partners with a number of organizations to explore and test cross-cutting improvement initiatives; we then share lessons learned with PCPI members and the broader health care delivery system. Based on findings from a survey of its members, PCPI creates QI training that meets the needs of practicing clinicians and their teams to drive quality improvement.

For example, based on learnings from the Closing the Referral Loop Project pilot program, PCPI, in collaboration with the Wright Center for Graduate Medical Education and the Pennsylvania Department of Public Health, will launch a toolkit that will help primary care physicians, specialty physicians and their staff improve processes related to physician-to-physician referrals; establish accountability for referrals; and improve information transfer using the electronic health record (EHR).

This groundbreaking work has led to PCPI working with a dedicated group of EHR vendors through the Office of the National Coordinator for Health Information Technology to develop a minimum standard to foster interoperability among disparate EHR platforms, and to facilitate closing the referral loop.
