PCPI Staffer Co-authors Transitional Care-cutting Inpatient Admissions for Vulnerable Adults Paper

Individually tailored transitional care (TC) may reduce hospital inpatient admissions for uninsured and Medicaid patients discharged with complex medical issues, says a study published in the May 2019 Journal of General Internal Medicine


Co-authored by PCPI’s Courtney Hurt, the study found that patients who received TC services including a comprehensive evaluation, education and support from social workers, clinicians and pharmacists had 42% fewer inpatient admissions during the 180 days after initial discharge.
